Clinical manifestations (include abnormalities in vital signs and physical examination) and course
Provide vascular access details, including location, type (Peripheral IV, PICC, UVC, UAC, PAL), date of placement +/- removal
Relevant laboratory tests (e.g., Blood gas, CBC, coagulation profile, CRP, cultures with organism identification, and antibiotic sensitivities). Provide test date, site of test collection for blood culture, and test result with units and reference range, if applicable
